Patient Policies & Forms

As a new patient, you can save time during your first visit to our office by printing and completing the necessary forms beforehand. This can be done at your convenience in the comfort of your own home before your scheduled appointment.

Referrals, Forms, Prescription Refills

Please allow sufficient time for these requests. Referrals must be registered with your insurance carrier and can take several days for us to get approval for you. Call us as soon as possible to start processing your referral.

To ensure the best possible care for any ongoing medical concerns, we kindly request a minimum of 24 hours notice for prescription refills. You can reach us between the hours of 9-5 to request a refill of your medications. Thank you.

Phone Calls:

Please don’t hesitate to call us. We want to ensure that you aren’t sitting at home or work worrying about anything without reaching out to us. No question is too small, and we want to make sure that you feel comfortable discussing any health concerns with us.

When calling our office after hours or on weekends, your call will be directed to our voicemail and answering service for emergency calls. To ensure your message is received clearly, please speak clearly and provide your phone number. Our voicemail system immediately notifies the physician of urgent messages, so please only leave a message if it specifically requires physician assistance. For issues such as prescription refills, non-urgent referrals, appointment scheduling, or administrative matters, we recommend contacting us during our regular weekday hours.

Billing:

Copayments, coinsurances, deductibles, and any charges for non-insurance-covered services must be paid at the time of service.

We must have updated confirmation of your insurance and identifying information at every visit. It is your responsibility to let us know of any changes in insurance data, addresses, telephone numbers, etc., and to ensure your insurance policy registered with LBHC remains updated and active.

We will bill the insurance company you designate for the services we provide. If we do not receive payment from your insurance company within 60 days, the balance transfers to your responsibility. You will be notified about any unpaid amounts.

Insurance:

At Live Better Medical Group, we accept most insurance plans. To ensure that you receive the best coverage, some insurance companies require you to choose a Primary Care Physician (PCP), use specific lab or radiology facilities, or see certain referral/specialty physicians when needed. Additionally, some insurers require preauthorization for certain services or prescriptions. We recommend that you contact your insurance company to determine if you have any such restrictions and inform us of any necessary information. Please note that different employers choose different insurance coverage, and we may not have all the specifics of your insurance plan. Therefore, it is your responsibility to ensure that your insurance and physician assignments are in order when you come to visit us.

Appointments:

First-time examinations are often lengthy visits because we need time to get to know each other. It is helpful to have all medical and immunization records sent to us PRIOR to the visit so these can be reviewed beforehand.

Cancellations / Missed Appointments:

If you need to cancel or reschedule an appointment, especially for a well visit or consultation, please give us at least 24 hours notice. These types of appointments require a significant amount of time, and we would like to be able to offer that time to other patients if you can’t make it. Please note that there is a $35 fee for routine appointments that are missed or canceled with less than 24 hours’ notice.
