Patient Policies & Forms

If you are a new patient visiting our office for the first time, you may save time by printing and filling out the following forms at your leisure, prior to arriving for your appointment. You can take your time, in the comfort of your own home.

Referrals, Forms, Prescription Refills

Please allow sufficient time for these requests. Referrals must be registered with your insurance carrier and can take several days for us to get approval for you. Call us as soon as possible to start processing your referral.

We ask for at least 24 hours notice for prescription refills so that we can review your chart and assure we are offering the best care for any ongoing problem. Call us to request a REFILL of your medications from 9-5.

Phone Calls:

We want you to call. We do not want anyone sitting at home (or at the office) worrying about something and not calling us. There is NO such thing as too small of a question, and we want you to feel comfortable with all issues regarding your health.

After hours and weekends, phone calls made to the office are received by voice mail and answering service for emergency calls. For those voicemail messages, it is important to speak clearly, especially when leaving your phone numbers. Our voice mail system always dispatches an urgent page to the physician, so please do not leave a message after hours that do not specifically need physician help. Issues involving refills, non-urgent referrals, appointment times, or administrative issues are best handled during our regular weekday hours.


Copayments, coinsurances, deductibles, and any charges for non-insurance-covered services must be paid at the time of service.

We must have updated confirmation of your insurance and identifying information at every visit. It is your responsibility to let us know of any changes in insurance data, addresses, telephone numbers, etc., and to assure your insurance policy registered with LBHC remains updated and active.

We will bill the insurance company you designate for the services we provide. If we do not receive payment from your insurance company within 60 days, the balance transfers to your responsibility. You will be notified about any unpaid amounts.


At Live Better Medical Group, we accept most insurance plans. Many insurances require that you choose a Primary Care Physician (PCP) and may require that you use a specific lab or radiology facility, or set of referral/specialty physicians when needed. Some insurers require preauthorization for certain services or prescriptions. Please contact your insurance company and find out if you have any such restrictions and let us know. Employers choose different insurance coverage, and we may not know the specifics of your insurance plan. It is your responsibility to assure that your insurance and physician assignments are in order when you come to visit us.


First-time examinations are often lengthy visits because we need time to get to know each other. It is helpful to have all medical and immunization records sent to us PRIOR to the visit so these can be reviewed beforehand.

Cancellations / Missed Appointments:

If you need to cancel or reschedule an appointment, particularly a well visit or consultation, please do so at least 24 hours in advance. A significant amount of time is allotted for these appointments and we would like to offer this time to other patients if you are unable to keep your appointment. There is a $35 fee for routine appointments missed or canceled with less than 24 hours notice.
