What should I bring with me?
Please bring your passport or driver’s license and vaccination records.
If you don’t have a copy of your vaccination records, please request your doctor or family to send a FAX of it to us at (727) 827-2809.
What does the exam include?
A review of all health & vaccine documents.
A complete medical history is taken.
All blood tests are required by Immigration.
Vaccination: The vaccinations required by the INS (Immigration and Naturalization Services) depend on your age. The INS (Immigration and Naturalization Services) provides a chart that explains which vaccines you need according to your age.
Forms and papers for INS (Immigration and Naturalization Services) will be completed and placed in a sealed envelope.
